A team led by University College London in the UK has reported findings that could help tailor treatment for high-risk prostate cancer without distant metastasis. The researchers examined whether Decipher, a molecular test performed on tumour tissue, could help guide the use of an additional hormone drug, abiraterone. Higher test scores were associated with worse outcomes in both datasets studied.
The analysis covered a real-world dataset of 27,607 men, with clinical outcomes analysed for 5,956. Researchers also examined 401 men with non-metastatic prostate cancer randomly assigned within the STAMPEDE abiraterone trial. Among men without lymph-node spread, the score predicted benefit from adding abiraterone to prevent distant spread.
Lead researcher Sarah Howlett said low-score patients might avoid intensified treatment, while those with high scores might be prioritised. However, she said further validation in other datasets was needed before the test could guide routine treatment decisions. Abiraterone can cause fatigue.
